You can complete using a smart phone (recommended), tablet or computer. All new users, please click the Register button. Once logged in you will have access to online services such as: Online Health Form Completion Online Immunization Completion Upload Insurance Information Receive Private Messages regarding your healthcare Receive Educational Material Print Copy of Immunizations North Carolina Law (General Stature 130A 152-157) requires documentation of immunizations within 30 days from the date of the student's first class. Failure to comply will result in your classes being CANCELLED. This law applies to all students except the following: students residing off campus and registering for any combination of: Off-campus courses Evening courses Weekend courses Medical Excemptions Religious Excemptions No more than four traditional day credit hours in on-campus courses Either have a medical provider sign the form to verify your immunization records and upload the verification or you can attach verified copies of your immunization record to your portal. All immunizations must be verified through an uploaded document of your choice. Acceptable copies of immunization records are high school records, medical records, military records or any form of immunization records with your name and DOB. We are committed to protecting your personal information. Data that you provide cannot be viewed by anyone else on the Web and is securely maintained by industry standard SSL (secure socket layer) encryption and decryption technology when needed.
